Why does my couch smell even though it looks clean?
Because it absorbs body oil every time somebody sits on it, plus spills, sweat and pet dander, all held deep in the fabric and the fill where you cannot see it. The surface can look fine while the fibres hold odour that releases when you press into the cushions. Fragrance only masks it.
The smell is below the surface
Fabric looks clean when the visible face is clean. The oils are in the weave and down in the foam, and the foam is where the volume is.
Press into a cushion and you push air out through material that has been collecting for years. That is the test, and it is why a couch can pass a visual inspection and fail a nose.
Animal oils are the hardest part
The microbiology oils animals leave behind produce that wet-animal, kennel smell in a room. Once that has built into the cushions it is very hard to get back out.
The reason is almost always time. People live with it, stop noticing it — olfactory adaptation is real and it happens within minutes — and call a year later. A couch cleaned at six months comes back well; the same couch at three years is a far harder job.
Why the spray makes it worse
Most fabric sprays are fragranced and sticky by design, and they go on with nothing to rinse them back out. What stays behind is residue, and residue holds soil, so the fabric gets dirty faster than it did before.
Odour control that works is subtractive. You take the source out of the cushion. Anything sprayed on top is covering something that is still there and adding a second problem underneath it.
The follow-up

It covers it for a few days and leaves you slightly worse off, because the residue holds soil. The fix is extracting the oils out of the fabric rather than adding something on top.
Olfactory adaptation. Your nose stops reporting a constant background odour within minutes, and a visitor’s has not adapted. It is also why a week away resets it completely.
