Subfloor contamination
Saturated subfloor swells and holds odour that no topical treatment can reach. At that point the correction is removing carpet and pad, sealing the subfloor with an odour barrier such as Kilz, and starting over. Two-story homes are worse because upstairs is wood subfloor, which absorbs.
What is actually happening
This is the most expensive failure mode in residential carpet, and it is entirely preventable with six-monthly inspections. Repeated marking against walls also swells baseboards and wicks into sheetrock.
How you prevent it
Keep animals off the second floor. Inspect every six months. Own a blacklight.
